Patients First Visit

If you are planning on visiting us for the first time, please arrive at least 15 minutes prior to your appointment time.

Please print and fill out the first three forms from our Patients tab, under Health Forms, and bring them in with you.

If you are planning on doing lab work with us, remember to fast 12 hours prior to your office visit and drink lots of water which helps the blood draw. Our phlebotomist is here from 7:00 A.M. to 11:30 A.M.

When you arrive at our location, parking will be available in the front and back of the building.  Take the elevator to the second floor and cross over on the walkway (bridge) to our building.

Our office enforces a no-show and cancellation policy of $60 for an office visit, $95 for a counseling visit, and $100 for Physical and CIMT visits.  All confirmed appointments must give notice 24 hours prior to cancellation.  If an appointment is made the same day, please call at least 2 hours before to cancel.  Thank you!
